Rank One is the FERPA- and HIPAA-compliant electronic medical records system that JPII and our partners at Youngs Physical Therapy have adopted to securely collect forms related to athlete eligibility and document athletic injuries. Both parents and students who have mobile phones should download the “Parent/Public Rank One” app.
Once you have downloaded the Rank One app, registered, and claimed your student(s), the forms below need to be uploaded/completed in Rank One prior to participation in practices or games. To do so, under “Your Students” tap the right arrow to the right of your student-athlete’s name and then tap on “View” next to “Forms.” A “X” will appear next to the forms that need to be completed. To complete the forms, tap “To Fill Out The Forms Click Here” at the bottom of the list of forms.
All forms listed on the Rank One Parent/Public app must be completed prior to the first official practice in order for an athlete to be listed as “eligible.”
In addition to the above, all students who wish to participate in a sport must have completed a Parent Athletic Permission Form (download) prior to the first official practice.
The North Carolina Independent School Athletic Association (NCISAA) requires that Pre-Participation Physical Examinations be completed every 13 months.
